What is rising damp?

Rising damp may be caused by a faulty damp proof course, or it could be the result of poor draining around your home. Modern homes are designed to deal with damp with proper drainage and ventilation, but these measures are known to fail, or they may have been installed incorrectly. Can rising damp be fixed?
